Instead of the chaos of scribbled and chaotic notes, or trying to remember numbers to report at your next check up, you can instead download these free printable blood pressure log sheets for consistent monitoring.
What’s included on the blood pressure log sheet?
It’s designed to be comphrensive and easy, with designated space to note important vitals. It’s great for home use, with a clean layout that makes it simple for both you and your healthcare provider(s).
At the top of the page, next to the “Health Tracker” label, you’ll find designated spaces to fill in your name and date of birth. This is useful if multiple members of your household are keeping track of blood pressure and heart rate. There are also spaces for date and time, so you can track exactly when you’ve taken your readings. There’s a space for blood pressure, with a slash in the center so you can note both your systolic and diastolic numbers.
I’ve also included a column for heart rate so you can record your pulse in order to help provide a more full picture of your cardiovascular health over time. Finally, there’s a column for additional remarks. Use this space to note things like what you ate, if you exercised, if you’ve missed a dose of medications, or if you’re feeling good or particularly unwell.
Why should I use printable health trackers?
You can hand your doctor a detailed and accurate log of daily readings, which can help them make more informed decisions about your treatment plan (if you need one). Doctors have limited time in appointments to review long term health, so when you walk in with a record of stats it can help them to see trends quickly. They might find patterns you don’t realise exist.
Note: this log is intended for personal record keeping purposes only. Always consult with your physician regarding your blood pressure readings, heart rate readings, and health decisions.
Printable in two sizes
To provide for both standards, I formatted this log sheet into two sizes: US Letter (for North American users) and A4 (for the rest of the world). Just click the relevant download link below. Once the pdf is downloaded, open it and print as many copies as you’d like.
